QLS Participant Fee & Card

We continue to appreciate your donation of $25.00 per year due September for the period September 2009 to June 2010. This $25.00 is used to defray the cost of meals and may be paid at the door after the meeting today. If you have paid your $25.00 donation, please check (at the front entrance) to see if your card is available.

Change of Personal Information

If your address or telephone number has changed recently, please fill out a "Change of Information" form in the Auditorium Corridor or Program Office.

QLS Has A Chiropractor and Podiatrist In The Wellness Center

Dr. Winston Carhee, Chiropractor, is available each Wednesday from 10:30 a.m. - 12:30 p.m.

New Medicare Benefit

Part C Medicare Advantage. If you have Diabetes, Congestive Heart Failure, End Stage Renal Disease, or COPD, (Chronic Bronchitis, Emphysema, etc.) you are eligible for Disease Management with Care Improvement Plus. Flyer are available on table in corridor.
